Here are some commonly needed Save Our Shores forms that you can download.  You will need a PDF reader such as Adobe to view and print these documents.

Forms:
General Beach Cleanup Waiver Form: This is our generic waiver form (some events require special waiver forms).  Participants need to turn in a signed waiver for all beach, river and SCUBA cleanups or storm drain stenciling events. All participants under age 18 must have a parent or guardian signature on their waiver.  You can print this form out, and bring it to an event already signed. 

Adopt a Beach Waiver Form:  Particpants in our Adopt a Beach Program must fill out an Application Packet which includes a specific Adopt a Beach waiver form.  The application packet will soon be available.

Annual Coastal Cleanup Day Waiver:  This waiver is used for all sites on Annual Coastal Cleanup Day only.  All participants under age 18 must have a parent or guardian signature on their waiver.  We highly encourage you to print this form out, and bring it to an event already signed to minimize your registration time.  Haga clic aqui por una exención de responsabilidad y asunción espresa del reisgo en espanol.
